Who Can Make a Referral?

We welcome referrals from:

NDIS participants
Family members and carers
Support Coordinators
Specialist Support Coordinators
Plan Managers
Local Area Coordinators (LACs)
Allied Health Professionals
General Practitioners (GPs)
Hospitals and Health Services
Community Organisations
Schools and Educational Providers
Aged Care and Disability Service Providers

Our Referral Process

Step 1 – Submit a Referral

Complete our referral form online, call our team, or send us an email with the participant's details and support requirements.

Step 2 – Initial Discussion

A member of our team will contact the participant or their nominated representative to discuss their goals, support needs, preferences, and NDIS funding.

Step 3 – Service Assessment

We review the participant's NDIS plan, required supports, availability, and service location to ensure we can provide the most appropriate assistance.

Step 4 – Service Agreement

Once supports have been agreed upon, we prepare a Service Agreement outlining the services, responsibilities, and funding arrangements.

Step 5 – Support Begins
