SUPPORT COORDINATION
WHAT IS SUPPORT COORDINATION?
Support Coordination is NDIS funded support designed to help you to make the most of your NDIS funds. Once your plan has been approved, you can work with a support coordinator to help you understand and implement your plan. Our focus is to work with you in optimising your plan so that you get the most out of your funded supports. A support coordinator can guide you through the NDIS process from start to end including:
- Working together to identify goals
- Connecting you with community, mainstream and other government services
- Selecting suitable support service providers based on your needs
- Budgeting for each type of support required and negotiations with service providers
- Ensuring service agreements and service bookings are completed
- Assisting you with preparing for your NDIS plan review
ABOUT LIBERTY CARE SUPPORT COORDINATION
Liberty Care is a registered provider of support coordination services under the National Disability Insurance Scheme (NDIS). If your NDIS plan includes coordination of supports, our friendly and experienced support coordinators can support you and your family throughout the process. We seek to provide the best possible services and support to enhance the lives of people with disabilities and their families.
Our Liberty Care Support Coordinators can work with you to build your skills and increase your capacity to live more independently by supporting you to engage in a mix of support services to:
- Maintain relationships
- Manage service delivery tasks
- Be included in your community
OUR SUPPORT COORDINATORS CAN HELP YOU WITH:
- Assessing the quality and value of your existing supports to ensure that they are meeting your needs
- Providing a range of supports which align with the support items in your NDIS plan that suits your individual needs across one or more providers
- Providing varying level of coordination and transition support, considering your circumstances and level of complexity
- Supporting you to connect with your community and encourage participation in both mainstream service and support options
- Building your capacity and networks with the aim of greater choice, control and independence in managing your personal lifestyle choices
